Choosing to do a research is a very personal decision that no one else can make for you. Like you, I scored a 258 on Step 1 (not AOA or GH). I chose dermatology very late (spring MS3 year) and was weak on research because most of my papers were submitted, not accepted/published at the time ERAS was due.

I would say getting an internal medicine letter is better than say something like a urology or radiology letter. I think having 2-3 dermatology research experiences with 2-3 citations out of each of those would be more than sufficient to get you competitive for dermatology as an applicant without a research year. That would be 2-3 projects with a paper, a poster, and an abstract with each of them, so like a total of 6-9 citations related to dermatology. I know Mohs surgeons making $300k working very leisurely, and I know Mohs surgeons making $10 million a year working like a factory machine. I'd take the average derm salary from those salary survey results for your region and add about $100k-$200k for roughly the same amount of "work." If you actually want to do research and can convey that, you will match to a 2+2 without a derm research year; it is a very small applicant pool that wants to do that, truly. The point of the derm research year in all honesty is to make connections, not to learn how to conduct research.
